چکیده
PATIENT Female, 28 FINAL DIAGNOSIS: Single intrauterine fetal death Symptoms: - MEDICATION - Clinical Procedure: - Specialty: Obstetrics and Gynecology. OBJECTIVE Rare disease. BACKGROUND Single fetal death in monochorionic twin pregnancy may result in poor perinatal outcome of the surviving twin, including neurologic sequelae, other organ injury, and death. In most reported cases of poor perinatal outcome in the surviving twin, monochorionic co-twin death occured after more than 20 weeks of gestation, while few with earlier occurrence have been presented. CASE REPORT A 28-year-old primigravid woman was referred to our hospital at 18 3/7 weeks of gestation for perinatal management of single fetal death in a monochorionic-diamniotic twin pregnancy. Our first evaluation by ultrasonography revealed a dead twin sized at 16 weeks of gestation, and an alive one with normal size and appearance, together with 1 placenta and 2 amniotic cavities with normal fluid amounts. At 20 3/7 weeks of gestation, ultrasonography showed that the surviving twin had bilateral ventriculomegaly and dilatation all around the subarachnoid cavity despite a normal head size. Fetal magnetic resonance imaging revealed remarkable atrophy of the cerebral cortex. After counseling, the patient and family chose termination of pregnancy, and artificial abortion was performed at 21 weeks of gestation. The aborted fetuses were not anomalous. Autopsy pathological findings confirmed encephalomalacia in the surviving twin. CONCLUSIONS Recent development of imaging device make it possible that several abnormalities in central nervous system can be detected in detail at earlier gestational age. It is important to keep this condition in mind even though single fetal death occurred at early gestational age.

A case of a surviving co-twin diagnosed with porencephaly and renal hypoplasia after a single intrauterine fetal death at 21 weeks of gestation in a monochorionic monoamniotic twin pregnancy
Monochorionic monoamniotic (MM) twin pregnancy carries a high risk of intrauterine fetal death (IUFD). Single IUFD in an MM twin pregnancy prior to 22 weeks of gestation has been reported to be strongly correlated with double twin demise. To our knowledge, there are no case reports on the natural course of a surviving co-twin in an MM twin pregnancy resulting in live birth after a single IUFD p...
متن کاملSingle Fetal Death in Monochorionic Twin Pregnancy: Co-Twin Prognosis and Neonatal Outcome.
The incidence of single fetal death in twin pregnancy varies from 0.5% - 6.8%, leaving the surviving fetus with increased morbi-mortality. The prognosis is worse in monochorionic pregnancies. In addressing these cases it should be noted referral to tertiary center with differentiated perinatal support, induction of fetal lung maturation and termination of pregnancy if there's loss of fetal well...

BACKGROUND We examined the perinatal outcomes in cases of at least one fetal demise in monochorionic and diamniotic twin pregnancies. METHODS We reviewed the obstetric records of all Japanese twin pregnancies managed beyond 9 weeks' gestation at Japanese Red Cross Katsushika Maternity Hospital between 2008 and 2014. RESULTS The incidence in the monochorionic twin pregnancies was significant...

I n the past 20 years, advancing maternal age and greater reliance on assisted-conception techniques have dramatically increased the incidence of multiple births, including monochorionic twins (see Glossary) [1,2]. Concomitantly, there have been advances in our understanding of these high-risk pregnancies. The offspring of multiple pregnancies are at greater risk for adverse perinatal outcomes ...
متن کاملEarly 2nd trimester fetal demise in a monochorionic twin pregnancy: A cautionary tale
It is well established that the death of one fetus in a monochorionic twin pregnancy places the surviving twin at significant risk for neuro-developmental delay or death. Although the early 1st trimester "vanishing twin" has not traditionally been considered a major risk, the precise gestational threshold beyond which a surviving twin is at risk remains uncertain. Most experts recommend serial ...
